DOSAGE AND ADMINISTRATION

Long test: Aldactone is administered at a daily dosage of 400 mg for three to four weeks. Correction of hypokalemia and of hypertension provides presumptive evidence for the diagnosis of primary hyperaldosteronism.

Short test: Aldactone is administered at a daily dosage of 400 mg for four days. If serum potassium increases during Aldactone administration but drops when Aldactone is discontinued, a presumptive diagnosis of primary hyperaldosteronism should be considered.

After the diagnosis of hyperaldosteronism has been established by more definitive testing procedures, Aldactone may be administered in doses of 100 to 400 mg daily in preparation for surgery. For patients who are considered unsuitable for surgery, Aldactone may be employed for long-term maintenance therapy at the lowest effective dosage determined for the individual patient.

NURSING MOTHERS

It is not known if fexofenadine is excreted in human milk. There are no adequate and well-controlled studies in women during lactation. Because many drugs are excreted in human milk, caution should be exercised when ALLEGRA is administered to a nursing woman.

WHAT IS NASACORT 55 MICROGRAMS/DOSE
NASAL SPRAY SUSPENSION USED FOR?

This medicine is used for the treatment of allergic conditions of the nose, which produce symptoms such as sneezing, itching, nasal stuffiness and runny noses. Nasal allergies which occur at any time of the year e.g. due to animal fur or house dust mites, are known as perennial allergic rhinitis. Nasal allergies which occur seasonally due to pollens (hayfever) are known as seasonal allergic rhinitis. This medicine will not relieve your symptoms immediately. It can provide relief from the first day of treatment but you will need to take it for a few days to reach maximum benefit.

BEFORE USING YOUR MEDICINE
● Do not use if you are hypersensitive to any of the ingredients. These are listed at the beginning of this leaflet.

● This medicine should not be used if you have any untreated fungal, bacterial or viral infections of the nose (e.g. herpes).

● This medicine should be used with caution if you have had recent surgery on your nose or recent prolonged nose bleeds or any other nasal problems.

● If your child has been prescribed this product for long term use this may have an effect on their growth rate. Your doctor will therefore regularly measure their height and if their growth rate is affected your doctor may reduce the dose prescribed.

● This medicine is not recommended for use in children under the age of 6 years.

● As with all medicines, you should consult your doctor before using this product if you are, or think you may be, pregnant, or if you are breastfeeding.

WHEN TO TAKE YOUR MEDICINE

Always take your nasal spray exactly as your doctor has told you to. For patients aged 12 years and older, the recommended dose is 2 sprays in each nostril, once daily. Once your symptoms are controlled you can reduce the dose to 1 spray in each nostril, once daily. For children aged 6-12 years, the recommended dose is 1 spray in each nostril, once daily. If the symptoms are not fully controlled at 1 spray in each nostril, once daily, it may be increased to 2 sprays in each nostril, once daily until symptom control is obtained. The dose can then be reduced again to 1 spray in each nostril, once daily. Do not use more than the recommended dose stated above. When this medicine is used in children under the age of 12 years, it is recommended that any period of continuous treatment should not exceed 3 months.

what is the maximum dosage of Tramadol that you can take a day?

The usual starting dose for chronic pain is 25 milligrams once a day in the morning. The daily dosage is then increased every 3 days until it reaches 200 milligrams taken in four doses of 50 milligrams each. After the phase-in period, Ultram may be taken in doses of 50 to 100 milligrams every 4 to 6 hours, depending on the severity of pain. If rapid pain relief is needed, the phase-in steps can be skipped, though side effects will be more likely. The maximum dosage under any circumstances is 400 milligrams a day (300 milligrams for those over age 75).

For people with kidney problems, the usual starting dose is 50 to 100 milligrams every 12 hours; and the maximum per day is 200 milligrams. For those with cirrhosis, the usual dose is 50 milligrams every 12 hours.

Adults (17 years of age and over)

For patients with moderate to moderately severe chronic pain not requiring rapid onset of analgesic effect, the tolerability of Tramadol can be improved by initiating therapy with the following titration regimen: Tramadol should be started at 25 mg/day qAM and titrated in 25 mg increments as separate doses every 3 days to reach 100 mg/day (25 mg q.i.d.). Thereafter the total daily dose may be increased by 50 mg as tolerated every 3 days to reach 200 mg/day (50 mg q.i.d.). After titration, Tramadol 50 to 100 mg can be administered as needed for pain relief every 4 to 6 hours not to exceed 400 mg/day. For the subset of patients for whom rapid onset of analgesic effect is required and for whom the benefits outweigh the risk of discontinuation due to adverse events associated with higher initial doses, Tramadol 50 mg to 100 mg can be administered as needed for pain relief every four to six hours, not to exceed 400 mg per day.

Individualization of Dose

Good pain management practice dictates that the dose be individualized according to patient need using the lowest beneficial dose. Studies with tramadol in adults have shown that starting at the lowest possible dose and titrating upward will result in fewer discontinuations and increased tolerability.

* In all patients with creatinine clearance less than 30 mL/min, it is recommended that the dosing interval of Tramadol be increased to 12 hours, with a maximum daily dose of 200 mg. Since only 7% of an administered dose is removed by hemodialysis, dialysis patients can receive their regular dose on the day of dialysis.
* The recommended dose for adult patients with cirrhosis is 50 mg every 12 hours.
* In general, dose selection for an elderly patient over 65 years old should be cautious, usually starting at the low end of the dosing range, reflecting the greater frequency of decreased hepatic, renal or cardiac function and of concomitant disease or other drug therapy. For elderly patients over 75 years old, total dose should not exceed 300 mg/day.

Seek emergency medical attention if you think you have used too much of Allegra.

Dizziness, drowsiness, and dry mouth have been reported with fexofenadine hydrochloride overdose. Single doses of fexofenadine hydrochloride up to 800 mg (6 healthy subjects at this dose level), and doses up to 690 mg twice daily for 1 month (3 healthy subjects at this dose level) or 240 mg once daily for 1 year (234 healthy subjects at this dose level) were administered without the development of clinically significant adverse events as compared to placebo.

If you took to much Allegra, consider standard measures to remove any unabsorbed drug. Symptomatic and supportive treatment is recommended. Following administration of terfenadine, hemodialysis did not effectively remove fexofenadine, the major active metabolite of terfenadine, from blood (up to 1.7% removed).

No deaths occurred at oral doses of fexofenadine hydrochloride up to 5000 mg/kg in mice (110 times the maximum recommended daily oral dose in adults and children based on mg/m² and up to 5000 mg/kg in rats (230 times the maximum recommended daily oral dose in adults and 210 times the maximum recommended daily oral dose in children based on mg/m². Additionally, no clinical signs of toxicity or gross pathological findings were observed. In dogs, no evidence of toxicity was observed at oral doses up to 2000 mg/kg (300 times the maximum recommended daily oral dose in adults and 280 times the maximum recommended daily oral dose in children based on mg/m².

Amoxicillin dosing instructions

Amoxicillin comes as a capsule, a tablet, a chewable tablet, a suspension (liquid), and pediatric drops to take by mouth. It is usually taken every 12 hours (twice a day) or every 8 hours (three times a day) with or without food. To help you remember to take amoxicillin, take it around the same time every day. Follow the instructions on your prescription label carefully, and ask your doctor or pharmacist to explain any part you do not understand. Take amoxicillin exactly as directed. Do not take more or less of it or take it more often than prescribed by your doctor.

Shake the liquid and pediatric drops well before each use to mix the medication evenly. Use the bottle dropper to measure dosing of pediatric drops. The pediatric drops and liquid may be placed on a child's tongue or added to formula, milk, fruit juice, water, ginger ale, or other cold liquid and taken immediately.

The chewable tablets should be crushed or chewed thoroughly before they are swallowed. The tablets and capsules should be swallowed whole and taken with a full glass of water.

Take amoxicillin until you finish the prescription, even if you feel better. Stopping amoxicillin too soon may cause bacteria to become resistant to antibiotics.

What is Antivert

Antivert is an antihistamine. It blocks the effects of the naturally occurring chemical histamine in your body.
Antivert is used to treat nausea, vomiting, and dizziness associated with motion sickness. Antivert may also be helpful in treating vertigo.
Antivert may also be used for purposes other than those listed in this medication guide.

Chemically, Antivert® (meclizine HCl) is 1-(p-chloro-α-phenylbenzyl)-4-(m-methylbenzyl) piperazine dihydrochloride monohydrate.

Inert ingredients for the tablets are: dibasic calcium phosphate; magnesium stearate; polyethylene glycol; starch; sucrose. The 12.5 mg tablets also contain: Blue 1. The 25 mg tablets also contain: Yellow 6 Lake; Yellow 10 Lake. The 50 mg tablets also contain: Blue 1 Lake; Yellow 10 Lake.

Based on a review of this drug by the National Academy of Sciences - National Research Council and/or other information, FDA has classified the indications as follows:

Effective: Management of nausea and vomiting, and dizziness associated with motion sickness.

Why is Aldactone prescribed?

Aldactone flushes excess salt and water from the body and controls high blood pressure. It is used in the diagnosis and treatment of hyperaldosteronism, a condition in which the adrenal gland secretes too much aldosterone (a hormone that regulates the body's salt and potassium levels). It is also used in treating other conditions that require the elimination of excess fluid from the body. These conditions include congestive heart failure, high blood pressure, cirrhosis of the liver, kidney disease, and unusually low potassium levels in the blood. When used for high blood pressure, Aldactone can be taken alone or with other high blood pressure medications.

Side effects cannot be anticipated. If any develop or change in intensity, inform your doctor as soon as possible. Only your doctor can determine if it is safe for you to continue taking Aldactone.

* Side effects may include:
Abdominal cramps, breast development in males, change in potassium levels (leading to such symptoms as dry mouth, excessive thirst, weak or irregular heartbeat, and muscle pain or cramps), deepening of voice, diarrhea, drowsiness, excessive hairiness, fever, headache, hives, irregular menstruation, kidney problems, lack of coordination, lethargy, liver problems, mental confusion, postmenopausal bleeding, severe allergic reaction, sexual dysfunction, skin eruptions, stomach bleeding, stomach inflammation, ulcers, vomiting

Special warnings about Aldactone

Potassium supplements or other diuretics that leave your potassium levels high, such as triamterene, should not be used while taking Aldactone, unless specifically indicated by your doctor. Symptoms of excess potassium include tingling sensations, fatigue, muscle weakness or paralysis, and a slow heartbeat. If you develop these problems, call your doctor immediately.

ACE inhibitors, used for blood pressure and heart failure, should not be taken while using Aldactone.

If you are taking Aldactone, your kidney function should be given a complete assessment and should continue to be monitored.

If you have liver disease, your doctor will be cautious about using this medication.

Excessive sweating, severe diarrhea, or vomiting could cause you to lose too much water and cause your blood pressure to become too low. Signs of dehydration include thirst, dry mouth, weakness, drowsiness, muscle fatigue, muscle cramps, restlessness, reduced urination, and a rapid heartbeat. Be sure to drink plenty of fluids whenever dehydration threatens, and be careful when exercising in hot weather.

Notify your doctor or dentist that you are taking Aldactone if you have a medical emergency, and before you have surgery or dental treatment.

Fluid Retention (Congestive Heart Failure, Cirrhosis of the Liver, or Kidney Disorders) Recommended dosage for Aldactone

The usual starting dosage is 100 milligrams daily either in a single dose or divided into smaller doses. However, your doctor may have you take daily doses as low as 25 milligrams or as high as 200 milligrams.

Your doctor may choose to adjust your dosage after an initial 5-day trial period or add another diuretic medication to this one.

Carcinogenicity (summary)

Studies of Aldactone (spironolactone) and the related compound potassium canrenoate (which, like Aldactone (spironolactone), metabolizes to canrenone) in rats for one to two year periods show an increase in carcinogenesis in the thyroid gland, testes, liver, breasts, and myelocytic leukocytes. Mammalian cells, depending on the presence of metabolic activation, show mixed results for mutagenicity in vitro.[6] In light of this research, Sandoz has recommended that unnecessary use of Aldactone (spironolactone) be avoided.

Orally administered Aldactone (spironolactone) has been shown to be a tumorigen in dietary administration studies performed in rats, with its proliferative effects manifested on endocrine organs and the liver. In an 18–month study using doses of about 50, 150 and 500 mg/kg/day, there were statistically significant increases in benign adenomas of the thyroid and testes and, in male rats, a dose–related increase in proliferative changes in the liver (including hepatocytomegaly and hyperplastic nodules). In a 24–month study in which the same strain of rat was administered doses of about 10, 30, 100 and 150 mg Aldactone (spironolactone)/kg/day, the range of proliferative effects included significant increases in hepatocellular adenomas and testicular interstitial cell tumors in males, and significant increases in thyroid follicular cell adenomas and carcinomas in both sexes. There was also a statistically significant, but not dose–related, increase in benign uterine endometrial stromal polyps in females.

A dose–related (above 20 mg/kg/day) incidence of myelocytic leukemia was observed in rats fed daily doses of potassium canrenoate (a compound chemically similar to Aldactone (spironolactone) and whose primary metabolite, canrenone, is also a major product of Aldactone (spironolactone) in man) for a period of one year. In two year studies in the rat, oral administration of potassium canrenoate was associated with myelocytic leukemia and hepatic, thyroid, testicular and mammary tumors.

Lamisil tablets and alcohol

The effectiveness of BuSpar in long-term use, that is, for more than 3 to 4 weeks, has not been demonstrated in controlled trials. There is no body of evidence available that systematically addresses the appropriate duration of treatment for GAD. However, in a study of long-term use, 264 patients were treated with BuSpar for 1 year without ill effect. Therefore, the physician who elects to use BuSpar for extended periods should periodically reassess the usefulness of the drug for the individual patient.

Atarax dose

For symptomatic relief of anxiety and tension associated with psychoneurosis and as an adjunct in organic disease states in which anxiety is manifested: in adults, 50-100 mg q.i.d.; children under 6 years, 50 mg daily in divided doses and over 6 years, 50-100 mg daily in divided doses.

Flexeril adult dose

Aldara cream effects on newborn

Pregnancy

Pregnancy Category C:

Systemic embryofetal development studies were conducted in rats and rabbits. Oral doses of 1, 5 and 20 mg/kg/day imiquimod were administered during the period of organogenesis (gestational days 6-15) to pregnant female rats. In the presence of maternal toxicity, fetal effects noted at 20 mg/kg/day [8X MRHD based on body surface area (BSA) comparisons] included increased resorptions, decreased fetal body weights, delays in skeletal ossification, bent limb bones, and two fetuses in one litter (2 of 1567 fetuses) demonstrated exencephaly, protruding tongues and low-set ears. No treatment related effects on embryofetal toxicity or teratogenicity were noted at 5 mg/kg/day (55X MRHD based on AUC comparisons).

Intravenous doses of 0.5, 1 and 2 mg/kg/day imiquimod were administered during the period of organogenesis (gestational days 6 18) to pregnant female rabbits. No treatment related effects on embryofetal toxicity or teratogenicity were noted at 2 mg/kg/day (1.5X MRHD based on AUC comparisons), the highest dose evaluated in this study, or 1 mg/kg/day (407X MRHD based on AUC comparisons).

A combined fertility and peri- and post-natal development study was conducted in rats. Oral doses of 1, 1.5, 3 and 6 mg/kg/day imiquimod were administered to male rats from 70 days prior to mating through the mating period and to female rats from 14 days prior to mating through parturition and lactation. No effects on growth, fertility, reproduction or post-natal development were noted at doses up to 6 mg/kg/day (87X MRHD based on AUC comparisons), the highest dose evaluated in this study. In the absence of maternal toxicity, bent limb bones were noted in the F1 fetuses at a dose of 6 mg/kg/day (87X MRHD based on AUC comparisons). This fetal effect was also noted in the oral rat embryofetal development study conducted with imiquimod. No treatment related effects on teratogenicity were noted at 3 mg/kg/day (41X MRHD based on AUC comparisons).

There are no adequate and well-controlled studies in pregnant women. Aldara Cream should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.

Nursing Mothers

It is not known whether imiquimod is excreted in human milk following use of Aldara Cream. Because many drugs are excreted in human milk, caution should be exercised when Aldara Cream is administered to nursing women.

Pediatric Use

AK and sBCC are not conditions generally seen within the pediatric population. The safety and efficacy of Aldara Cream for AK or sBCC in patients less than 18 years of age have not been established.

Safety and efficacy in patients with external genital/perianal warts below the age of 12 years have not been established.

Aldara Cream was evaluated in two randomized, vehicle-controlled, double-blind trials involving 702 pediatric subjects with molluscum contagiosum (MC) (470 exposed to Aldara; median age 5 years, range 2-12 years). Subjects applied Aldara Cream or vehicle 3 times weekly for up to 16 weeks. Complete clearance (no MC lesions) was assessed at Week 18. In Study 1, the complete clearance rate was 24% (52/217) in the Aldara Cream group compared with 26% (28/106) in the vehicle group. In Study 2, the clearance rates were 24% (60/253) in the Aldara Cream group compared with 28% (35/126) in the vehicle group. These studies failed to demonstrate efficacy.

Similar to the studies conducted in adults, the most frequently reported adverse reaction from 2 studies in children with molluscum contagiosum was application site reaction. Adverse events which occurred more frequently in Aldara-treated subjects compared with vehicle-treated subjects generally resembled those seen in studies in indications approved for adults and also included otitis media (5% Aldara vs. 3% vehicle) and conjunctivitis (3% Aldara vs. 2% vehicle). Erythema was the most frequently reported local skin reaction. Severe local skin reactions reported by Aldara-treated subjects in the pediatric studies included erythema (28%), edema (8%), scabbing/crusting (5%), flaking/scaling (5%), erosion (2%) and weeping/exudate (2%).

Systemic absorption of imiquimod across the affected skin of 22 subjects aged 2 to 12 years with extensive MC involving at least 10% of the total body surface area was observed after single and multiple doses at a dosing frequency of 3 applications per week for 4 weeks. The investigator determined the dose applied, either 1, 2 or 3 packets per dose, based on the size of the treatment area and the subject's weight. The overall median peak serum drug concentrations at the end of week 4 was between 0.26 and 1.06 ng/ml except in a 2-year old female who was administered 2 packets of study drug per dose, had a Cmax of 9.66 ng/mL after multiple dosing. Children aged 2-5 years received doses of 12.5 mg (one packet) or 25 mg (two packets) of imiquimod and had median multiple-dose peak serum drug levels of approximately 0.2 or 0.5 ng/mL, respectively. Children aged 6-12 years received doses of 12.5 mg, 25 mg, or 37.5 mg (three packets) and had median multiple dose serum drug levels of approximately 0.1, 0.15, or 0.3 ng/mL, respectively. Among the 20 subjects with evaluable laboratory assessments, the median WBC count decreased by 1.4*109/L and the median absolute neutrophil count decreased by 1.42*109/L.

Bentyl dosage

DOSAGE MUST BE ADJUSTED TO INDIVIDUAL PATIENT NEEDS

Adults-Oral. The only oral dose clearly shown to be effective is 160 mg per day (in 4 equally divided doses). Since this dose is associated with a significant incidence of side effects, it is prudent to begin with 80 mg per day (in 4 equally divided doses). Depending upon the patient's response during the first week of therapy, the dose should be increased to 160 mg per day unless side effects limit dosage escalation. If efficacy is not achieved within 2 weeks or side effects require doses below 80 mg per day, the drug should be discontinued. Documented safety data are not available for doses above 80 mg daily for periods longer than 2 weeks.

Adults-Intramuscular Injection. NOT FOR INTRAVENOUS USE.

The intramuscular dosage form is to be used temporarily when the patient cannot take oral medication. Intramuscular injection is about twice as bioavailable as oral dosage forms; consequently, the recommended intramuscular dose is 80 mg daily (in 4 equally divided doses). Oral dicyclomine hydrochloride should be started as soon as possible and the intramuscular form should not be used for periods longer than 1 or 2 days.

ASPIRATE THE SYRINGE BEFORE INJECTING TO AVOID INTRAVASUCLAR INJECTION, SINCE THROMBOSIS MAY OCCUR IF THE DRUG IS INADVERTENTLY INJECTED INTRAVASCULARLY. Before using Bentyl Syrup:

Some medical conditions may interact with Bentyl Syrup. Tell your doctor or pharmacist if you have any medical conditions, especially if any of the following apply to you:

* if you are pregnant, planning to become pregnant, or are breast-feeding
* if you are taking any prescription or nonprescription medicine, herbal preparation, or dietary supplement
* if you have allergies to medicines, foods, or other substances
* if you have chronic inflammation or ulcers of the bowel, diarrhea, enlargement of the prostrate, hiatal hernia, kidney or liver disease, nerve problems or disease, overactive thyroid, or urinary blockage or retention
* if you have heart disease, heart failure, a history of rapid or irregular heartbeat, or high blood pressure
* if you are at risk for glaucoma
